การวิจัยครั้งนี้มีวัตถุประสงค์เพื่อประเมินระดับความรู้ความเข้าใจ ทัศนคติ และความไว้วางใจของผู้บริโภคที่มีต่อนโยบายการร่วมจ่ายแบบมีเงื่อนไข (Conditional Co-payment) และวิเคราะห์ปัจจัยที่มี
ความสัมพันธ์ต่อการตัดสินใจซื้อหรือต่ออายุ... โดยเก็บข้อมูลด้วยแบบสอบถามจากลุ่มตัวอย่างผู้บริโภคในจังหวัดเชียงไหม จำนวน 400 คน ดวยวิธีการสุมตัวอยางแบบตามสะดวก (Convenience Sampling) และวิเคราะห์ด้วยสถิติเชิงพรรณนา แบบจำลองสมการถดถอยโลจิสติกแบบทวิภาค พร้อมทั้งตรวจจสอบความเหมาะสมของแบบจำลอง (Goodness-of-ht) และการวิเคราะห์ผลกระทบส่วนเพิ่ม (Marginal Effects) ผลการศึกษาพบว่า... ปัจจัยที่มีความสัมพันธ์เชิงบวกต่อการตัดสินใจซื้อหรือต่ออายุ ได้แก่ ความไว้วางใจในความโปร่งใสของบริษัทประกัน (Marginal Effect = 0.251, p < 0.001) ทัศนคติต่อความเสี่ยง และการมีกรมธรรม์เดิมอยู่แล้ว ในขณะที่ปัจจัยที่มีความสัมพันธ์เชิงลบ ได้แก่ เพศชาย พฤติกรรมการพบแพทย์บ่อยครั้งในอดีตและสถานะสุขภาพใจในเกณฑ์ปกติ (Marginal Effect = -0.005, p = 0.004) ข้อมูลเชิงประจักษ์ที่สะท้อนความสัมพันธ์เหล่านี้ นำไปสู่ข้อเสนอแนะเชิงนโยบายต่อสำนักงานคณะกรรมการกำกับและส่งเสริมการประกอบธุรกิจประกันภัย (คปภ.) ในการมุ่งเน้นสร้างความเชื่อมั่นผ่านระบบฐานข้อมูลสาธารณะเพื่อแสดงสถิติการเคลม และการบูรณาการข้อมูลร่วมกับสำนักงานหลักประกันสุขภาพแห่งชาติ (สปสช.) เพื่อเฝ้าระวังปรากฏการณ์การไหลกลับของกลุ่มผู้ป่วย (Reverse Adverse Selection) ที่อาจตัดสินใจออกจากระบบประกันภาคเอกชนและไหลกลับเข้าสู่ระบบสาธารณสุขของรัฐ ซึ่งอาจก่อให้เกิดผลกระทบภายนอกทางการคลัง

บทคัดย่อ (อังกฤษ)

This study aims to assess consumers' knowledge, attitudes, and trust regarding the Conditional Co-payment policy, and to analyze the factors associated with the decision to
purchase or renew private health insurance under these conditions. Data were collected via questionnaires from a sample of 400 consumers residing in Chiang Mai province using
convenience sampling. The data were analyzed utilizing descriptive statics and a Binary Logistic Regression model, incorporating model diagnostics for goodness-of-fit and Marginal
Effects analysis. The findings revealed that factors positively associated with the decision to purchase or renew included trust in the transparency of insurance companies (Marginal Effect = 0.251, p < 0.001), risk attitudes, and possession of an existing insurance policy. Conversely, male gender, a history of frequent medical visits, and a normal mental health
status exhibited a negative association (Marginal Effect = -0.005, p = 0.004). The empirical evidence reflecting these associations yields policy recommendations for the Office of
Insurance Commission (OIC) to prioritize building consumer trust through the establishment of a public database displaying claim statistics. Furthermore, data integration with the
National Health Security Office (NHSO) is recommended to monitor the phenomenon of Reverse Adverse Selection, wherein high-utilization patients may opt out of the private
insurance system and return to public healthcare, potentially generating long-term fiscal externalities.
